Client agreement and subscription terms
This plan is not an insurance policy. It is a plan to provide optimal wellness care and preventive treatment for Patient at one or more hospitals operated by National Veterinary Associates, Inc. (“NVA”).
This plan is not transferable to another pet or assignable by You to another person.
You or NVA may cancel this agreement at any time. If You cancel (including for the death of Patient), whether before or after services have been rendered, NVA shall be entitled to retain the entire initial membership fee and shall be entitled to retain or recover from You all monthly installments which have become due, including the installment for the month in which the cancellation occurs. If the total amount of services rendered by NVA prior to cancellation, (valued at hospital's full retail fee schedule pricing), exceed the sum of monthly installments retained or recovered by NVA, You shall be obligated to do one of the following, as selected by You: (a) immediately pay for services in excess of the payments received; (b) immediately pay in full the remaining monthly installments for the remainder of the term.
This agreement has a one-year term starting on the Plan Start Date and automatically renews for additional one-year terms unless and until You or NVA terminate this agreement with at least 30 days written notice before the end of the current term. Membership fees are a one-time charge and are not charged in a renewal term. Adult plans will automatically renew into the existing service plan level. Puppy/Kitten Comprehensive plans will automatically renew into the Adult Core plans.
NVA reserves the right to increase plan fees upon any renewal.
The additional discounts on veterinary services that You are eligible to receive in addition to Your discounted plan services exclude all pharmacy, prescription and non-prescription flea control products, prescription heartworm preventives, diet and supplements, retail goods, boarding, and grooming.
You shall not be entitled to a refund for any unused pre-paid fees.
You may pay the annual plan fee in full at enrollment or in monthly installments. If electing monthly installments, the first payment is due at enrollment.
Plan installment payments can either be billed directly to your credit card or deducted from Your bank account via a debit card transaction. You may be billed a $25 overdraft and reprocessing fee on returned charges. You are responsible for notifying NVA of any changes to billing accounts. In the event You fail to pay any installment, NVA may immediately suspend plan membership and all remaining eligible services until the balance of payments owed is made current, and you may be charged a late fee.
Please note that certain states impose sales/use tax on the provision of veterinary services. Monthly payments for plans in these states will have sales tax reimbursement added to monthly charge. However, under these various states’ laws, it is required that NVA report and remit sales taxes based on the full annual valueof the plan at the initiation of the plan. The sales tax added to the monthly charge is simply to reimburse NVA for sales taxes that have already been remitted to the taxing jurisdiction.
